Competition's impact: a report from the front.

چکیده
During the 1980s, the United States veered away from an entitlement health policy framework toward the unleashing of competitive forces in health insurance and services. In a health system as vast as the American system, disaggregating the multiple impacts of this revolutionary movement is no easy task. But competition has significantly altered the health care landscape in only five years, confronting policymakers with new challenges. Though many observers believe that Medicare payment reforms were the principal catalyst of market change, the root causes run far deeper. Public and private health care financing reforms stemmed from the collision of hyperinflation in health costs following the 1979 oil shock with the deep recession of 1981–1982. This collision produced a series of weakening aftershocks that lasted well into the decade. The recession resulted in a severe fiscal crisis both for state and federal governments and led not only to Medicare prospective payment but to tightening of Medicaid eligibility and payments as well. However, more significantly, the recession forced a fundamental restructuring of private health coverage, reversing almost three decades of steadily expanding benefits.

Assessment and Design Enhancement of the Front End Energy Absorption Mechanism of a Locomotive Based on an Impact Scenario
This research is concerned with weighing the behavior of the front end energy absorption system of a locally manufactured locomotive in crash situations. The causes of the extensive damages to the energy absorption apparatus that includes the crash element and the buffer are studied. By choosing the proper damage model the conditions of the accident is simulated by using the ABAQUS engineering ...

Thin-walled structures like crash boxes may be used as energy absorption members in automotive chassis. There have been many studies addressing the behaviors of energy absorption members on frontal crash. These researches have attempted to predict the energy absorption and maximum impact load in shell structures. The energy absorption and maximum impact load depend on many parameters including ...

متن کاملCompetition and health care: planning for deregulation.
T WAS inevitable that Congress's deregulatory right hand would eventually challenge what its left hand had been doing for years to the health-care industry-gradually tightening the government's regulatory grip. The first evidence of the challenge was a series of amendments, adopted last October, to the National Health Planning and Resources Development Act of 1974. These amendments significantl...
